Service Description

Upon booking, you will receive an email with instructions as well as an NAQ and informed consent to be filled out and returned. These assessments take place over email. We recommend adding this service onto your assessment to get a better picture of any underlying health issues. You can also book this separately as a consultation to assess bloodwork (also hair analysis, urinalysis, saliva analysis, and/or stool analysis) independently of an assessment or any other service. Please include all current testing. We also like to establish a baseline, so if you have testing from previous years, we like that as well.

Cancellation Policy

If, for any reason you must cancel your appointment, please let us know at least 48 hours ahead of time. If, for any reason you must cancel your membership, please send notice to our office or your trainer/specialist notifying them of the cancellation. All fees must be paid for the month (if you have already begun the month) and cannot be refunded.
